RESUMO

Present study was conducted on prostaglandin F2alpha (PGF2alpha), oxytocin, (OT), potassium chloride (KCI) and barium chloride (BaCl2) pre-contracted perimetrial uterine strips of dioestrus and pregnant buffaloes to evaluate the tocolytic efficacy of selective beta2 adrenoceptor agonists-albuterol (salbutamol) and terbutaline. Cumulative concentration-response curves of both the beta2 adrenoceptor agonists were constructed and the mean effective concentration (EC50) values determined and compared statistically. Based on the comparative EC50 values in relaxing the pre-contracted uterine strips with different spasmogens, the rank order potency of albuterol was found to be--PGF2alpha > BaCl2 > OT > KCl on uterine strips from dioestrus animals, while OT> BaCl2> PGF2alpha >KCl on the uterine strips of pregnant buffaloes. The rank order potency of terbutaline on uterine strips from dioestrus stage animals was- BaCl2 > OT > KCl > PGF2alpha, while BaCl2 > PGF2alpha > KCl > OT on uterine tissues of pregnant animals. Thus, irrespective of the state of uterus, whether gravid or non-gravid, KCl-depolarized uterine tissues required comparatively higher concentrations of albuterol or terbutaline to produce tocolytic effect. High concentrations of K+ in biophase may have interfered with the beta2 adrenoceptor agonists-induced outward K+ current and hyperpolarization. From the results of present study, it was evident that selective beta2 adrenergic agonists had good tocolytic efficacy on the uterus of buffaloes. Further, indirectly the possibility of existence and activation of K(Ca) channels by selective beta2 adrenoceptor agonists in mediating tocolysis of buffalo myometrium can not be ruled out, however, detailed studies using specific K(Ca) channel blockers are required for characterizing the nature of such channels in buffalo uterus.

This study assessed the effects of IV terbutaline [Bricanyl] as a prototype of beta 2 mimetics on cardiorespiratory function in 20 patients with COPD [16 males, 4 females]. Arterial blood gases on admission showed a mean PO2 of 124 mmHg and a mean PCO2 of 59 mmHg. 14 out of 20 patients were on assisted ventilation. All patients were subjected to cardiac catheterization using flow directed Swan Ganz catheter. Baseline evaluation was followed by hemodynamic measurements. Beta mimetic therapy increased HR by 23%, BP by 11% and PCWP by 27%, while there was almost no effect on CVP. Intravenous Bricanyl significantly augmented CI, SVI and increased LVSWI and RVSWI. IV Bricanyl substantially increased SaO2, SVO2, O2 transport and O2 uptake without significantly changing O2 extraction ratio. The increase in the SVSWI was highly significant compared with the modest increase in LVSWI

RESUMO

En los últimos 7 años algunos estudios han establecido que la Furosemida nebulizada inhibe la broncoconstricción inducida por metabisulfito, ejercicio, aguda destilada y alergenos. Hay poca información sobre la utilidad clínica de la Furosemida y no hay experiencia en Crisis Asmática (CA). Para resolver tales interrogantes se realizó el presente estudio. Los pacientes con CA que consultaron al servicio de urgencia del Hospital Universitario de Cartagena desde noviembre de 1993 hasta mayo de 1994 ingresaron en el presente estudio, previo consentimiento informado. A todos los pacientes se les realizó historia clínica y cada hora se monitorizaron: signos vitales, pulso paradójico, cianosis, estado de conciencia, sibilancias, tirajes y Flujo Espiratorio Pico (FEP). Los pacientes con CA severa o Asma Potencialmente Fatal fueron excluidos. Al ingreso, al azar y doble ciego, los pacientes pasaron a un grupo terapéutico de Terbutalina ( 2 gotas por cada 10 Kg de peso Solución nebulizadora de 1cc=10mg) o de Furosemida (40 mg nebulizados). En ambos grupos la nebulización se realizó cada 20 minutos por 3 ocasiones, la primera hora, y luego cada 4 horas durante un máximo de 24 horas. En CA moderada se asoció aminofilina IV en ambos grupos. Cada hora se analizaron los parámetros anotados y según ellos se decidió hospitalizar, continuar manejo en el servicio de urgencia o dar de alta. Para el análisis estadístico se registraron los datos en una tabla maestra, se tomaron medidas de tendencia central (media), porcentajes, desviación estándar. Los parámetros clínicos y funcionales evaluados fueron analizados mediante prueba T de student (p menor 0.05).

RESUMO

Pulmonary surfactant activity of healthy male albino rats was estimated in terms of the maximum and minimum surface tension values of alveolar washings and the phospholipid content of the extract. The results obtained in these (control) animals were compared with those in three groups of animals treated with therapeutic doses of terbutaline, adrenaline and aminophylline. A significant decrease in the surface tension values without a significant increase in the phospholipid content was observed with aminophylline, whereas a significant increase in phospholipid concentration without a significant decrease in surface tension values was observed in case of terbutaline and adrenaline. These findings suggest that aminophylline, in addition to a bronchodilator action, lowers the elastic resistance of lung. The study also indicates caution in interpreting phospholipid concentration as surfactant activity.

RESUMO

This study was done in a trial to know the effect of terbutaline, given subcutaneously in a dose of 0.005 mg/kg, to asthmatic children during attacks. Two groups were studied: Group I: consisted of twenty patients [12 males and 8 females] with bronchial asthma during attacks. Their ages ranged from 5-12 years, and Group II: consisted of ten normal healthy children of an analogous age, sex and height. They were taken as a control group,. Ventilatory function tests [FEVI, FVC, FEVI%] were measured in both groups. In group I, the measurement was done before an half an hour after terbutaline injection. Serum immunoglobulins IgG, IgM, IgA and IgE were measured in both groups. In group I, the measurement was done before and also half an hour after terbutaline injection. The results of the study showed that: [1] there was marked subjective and objective clinical response to subcutaneous terbutaline, [2] there was no subjective or objective side effects to terbutaline, [3] Ventilatory function tests [FEV1, FVC, FEV1%] improved significantly [P < 0.001], [4] IgG decreased insignificantly [P > 0.05], [5] IgA increased significantly [P < 0.01] after terbutatline, [6] IgM did not differ significantly after terbutaline [P > 0.05], and [7] IgE decreased significantly after injection [P < 0.001]
